This book contains lists of names of individuals (both living and deceased) that families in the parish wished to have commemorated during church services. In most cases, the given name and surname of the head of the household is at the top of the page, followed by only the given names of those to be commemorated.

Fragment of the church altar with a hand-written inscription commemorating its consecration by Bishop Bohdan (Shpylka) and the cavity for the deposition of relics.
